Search Filters of H11 Report
Choose the Specialty, Legal Entity, Provider, Type of Service (New Patient/Established Patient), year on which Medicare data is based, Report Type (Units/Payments), and the Date of Service range.

| Specialty | Select one from the drop-down; |
| Legal Entity | Select an LE to show results pertaining to that LE |
| Provider | Select a provider to show results specific to the provider |
| Type of Service | Choose either New or Established patient |
| Medicare data based on | Select the year |
| Report Type | Units/Payments; Default is Units |
| Date of Service range | Choose the DOS range |
